Abstract

Community Health Center is a basic health care facilitiy for the community. Community health center commonly uses papers to write medical records. Medical Records is a recording activity of information, anamnesis, diagnosis and procedure, as well as patient treatment records. The disadvantages of this recording method are the difficulty of grouping medical records, large storage is needed, the durability of storage media, lack of data backup, and slow data access. One of the solutions to solve the above problems is developing a medical record information system. This research aims to design and build information systems that is able to record patient registration, saving medical records data and providing information about the history of the patient's medical record, and helping medical officers in performing their duties. This system was developed using method of SDLC (Software Development Life Cycle), namely: phase analysis, design, implementation and testing. The system has been built using PHP programming language and MySQL DBMS (Data Base Management System). The result of this research is a web-based medical record information system that is able to manage patient data, treatment data, and outpatient medical records. After performed beta testing using blackbox testing method, the results system functionality testing has been obtained that is 70% strongly agree and 30% agree. The interface system testing resulted that the 44% strongly agree, 54% agree, and 2% disagreed. The system benefits testing resulted that the 42.5% said strongly agree and 57.5% agree. It can be concluded that the functionality of the system running properly, the system interface is easy to use, and the system is relatively helpful.
