The research was aimed to know maximum ground vibration acceleration value used to make maximum ground vibration acceleration value map and model earthquake based on maximum ground vibration acceleration value at Sermo Dam in order to get earthquake risk model in dam area. Data collection was done by using Seismometer TDV-23S with 11 measuring spot. Result of the data was analyzed using Horizontal to Vertical Spectral Ratio (HVSR) method to get predominant frequency value and amplification factor in every measuring spot. Software which used are DataPro, Sessaray-Geopsy, Google Earth, and Surfer 11. PGA value calculation was using Kanai empiric equality. Earthquake event was chosen 6 events from the dam to get maximum PGA and was using 33 earthquake events which radius was 10 km from it. Event with biggest magnitude was used to earthquake modeling based on PGA value. Modeling result gathered 10 PGA microzonation map and in modeling with 5.0 SR magnitude 10 km depth was got maximum PGA value as 149.1730 gal with high risk on left dam support. %Z Pembimbing : Nugroho Budi Wibowo, M.Si
