Assessment and evaluation for the teachers needed to measure these aspect as a benchmark of the institution progress. Therefore, instructor of ICT training UIN Sunan Kalijaga Yogyakarta require some ratings that are directly voted by the trainees. Extreme Programming methodology used in this research as a software development process. This method has a simple process, quick and precise because involves the client to interact directly during the software development process. Extreme Programming life cycle consists of planning, designing, coding and testing. The refactoring process makes this method as an adaptive software development methodology. ICT training instructor performance assessment system has features to manage the schedules, instructor and the trainees. This system can be accessed by the trainees to undertake an assessment of instructors. This system helps the ICT training manager to find out the instructor performance as an evaluation and determination of future employment contracts. The results of functionality testing from this system is 100\%, while for usability as much as 98.87\% of respondents accept this system.} }
