This research is R&D with procedural model. By using 4-D model with modification, wich are (1) Define (2) Design (3) Develop (4) Disseminate, the research has been done. This research used assessment sheet for validator; and question sheet to assess student’s misconceptions. The quality of instrument test used 4 likert scale in checklist form. Subject in this research is 10th student of State Senior High School 2 Bangutapan Bantul academic year 2015/2016 at testing of natural science 3 on 10th grade and natural science 1 on 10th grade as disseminate of product. Result of this research are obtained instrumen test CSRM (Conceptual Survey of Rectilinier Motion) based on representation of charts with three tier test to assess student’s misconceptions about rectilinier motion. This instrument test consist of 15 questions come with instruction about the use, time allocation, and analysis of answer. Quality of product based on assessment of physics expert has a good score, evaluation expert has very good score, and teacher has very good score, with average score are 3.09, 3,55, and 3,46. The average misconceptions identified 47,6 % in rectiliner motion concept. %Z Drs. Nur Untoro, M. Si
