        abstract = {This study aims to observe, analyze and draw conclusions about the
governance records retrieval of dynamic medical records archive for easy access
in thePolyclinicof UIN Sunan Kalijaga Yogyakarta. This study used descriptive
qualitative method. Data collection techniques using the method of observation,
interviews, and documentation. Analysis of experimental data using three
stages, namely data reduction, data presentation, and verification. Test the
validity of the data using an extension of observation, triangulation, and
membercheck.
From the results of this research,it?s knownthat the governance of
dynamic medical records archive in the Polyclinic of UIN Sunan Kalijaga was
guided by the Regulation of the Minister of Health No.
269/Menkes/Per/III/2008 aboutmedical records and Hospital medical records
management guidelines in Indonesia that issued by the Ministry of health of
Republic of Indonesia. Processing system of dynamic medical records archive at
the Polyclinic of UIN Sunan Kalijaga Yogyakarta using a numeric system
whichis managing or storing the archive by number.Management process that
conducted in the Polyclinic ofUIN Sunan Kalijaga started with the creation
process and procurement records, assembling, coding, indexing, and filing.
User access to medical records archive at the Polyclinic UIN Sunan Kalijaga is
easy but limited. User can access archived medical records only if they seek
treatment at the Polyclinic UIN Sunan Kalijaga and for important matters such
as law enforcement, agency requests under the Act or for research and educat ion
in accordance with the Regulation of the Minister of Health No. 269 / Menkes /
Per / III / 2008 on Medical Record article 10. In the process, there are three
obstacles, namely: the standard procedures of governing the medical records is
not yet drawn up, lack of closed shelf and a separate room for archival storage
of medical records and lack of human resources.}
