This makes frozen food processing a high-risk industry. Based on observations carried out on CV Fania Group, show supply chain risks such as unstable milkfish supply and unsterile production processes. Therefore, corrective action is needed Failure Mode and Effect Analysis (FMEA) are used to identify and analyze risks based on severity, frequency of occurrence, and level of detection. These three criteria are used to determine the value of the Risk Priority Number (RPN), the higher the value, the more problematic the risk and need to be addressed immediately. The priority risks for this company are unstable milkfish supply with RPN 376,158, delayed supply of milkfish with RPN 263,481, damaged products during storage with RPN 261,888, and unsterilized production process with RPN 240,016. The case studies suggest that the company increases the number of suppliers, evaluates supplier performance, monitors Standard Operating Procedures (SOP), supervise production activities, controls product quality, expands production sites, makes production station plans that pay attention to contamination, plans production quantities, provide refrigerators for retail, and control storage temperatures.
