Effective risk management facilitates informed decision-making through the identification, assessment, classification, and subsequent formulation of mitigative strategies to oversee and curtail these risks. This study examines supply chain risk management at Tofu Manufacturer X in Kediri. This study employs the Supply Chain Operation Reference (SCOR) model, which encompasses five business processes: plan, source, make, deliver, and return, to identify risk events within the supply chain. Additionally, the research utilizes the House of Risk (HOR) method, comprising two stages: HOR 1 and HOR 2. HOR 1 is utilized to determine the Aggregate Risk Potential (ARP) value, while HOR 2 focuses on establishing mitigation strategies priorities. A Pareto diagram is also employed to evaluate and prioritize risk agents based on their ARP values. The study successfully identified 24 risk events and 15 risk agents at Tofu Producer X. Following the identification, a risk evaluation was conducted, resulting in 8 prioritized risk agents that needed addressing, which then formed the basis for formulating 13 preventive action proposals. The research further discusses the managerial implications for Tofu Producer X, particularly concerning the benefits and implementation of the proposed mitigation strategies.} }
