abstract: Azhar Songket UKM is a creative industry engaged in traditional weaving crafts,
especially songket cloth production. Songket cloth is a symbol of Malay cultural
identity. In its preservation, it is often used in traditional events, weddings, and
cultural ceremonies, not only songket cloth, Azhar Songket UKM also produces
scarves, tengkuluk caps (head coverings). Based on field observations, this UKM
faces a number of challenges in supply chain activities such as late arrival of raw
materials due to dependence on one supplier, obstacles in the production process
that result in late delivery of final products to customers. This study focuses on
identifying various potential risks that disrupt the smooth supply chain of songket
cloth at Azhar Songket UKM and designing mitigation steps to prevent and
minimize the potential for these risks. The approach used is the Supply Chain
Operations reference (SCOR) which is used to classify supply chain activities into
five processes, namely plan, source, make, delivery, and return. Furthermore, the
House of Risk (HOR) method is used to identify and prioritize risk agents and design
appropriate mitigation strategies. From the research results, 22 risk events and 45
risk agents were found. Based on the Pareto 80/20 approach, 22 risk agents were
selected as the main focus of mitigation. Furthermore, 22 mitigation strategies were
designed to minimize risks in the supply chain process of Azhar Songket SMEs.
