In facing tight business competition, efficient supply chain management is the key to a company's success in maintaining operational continuity and maintaining competitive advantage. One of the main challenges in supply chain management is the risk of late delivery of finished products, which can hamper the production process and reduce customer satisfaction levels. CV 1Souvenir Indonesia, a company engaged in the manufacture of plaques based on a make-to-order system, often experiences problems with late delivery of finished products to customers. The level of delay reaches a percentage of up to 60%, which has a significant impact on the company's operational performance and reputation. Therefore, a systematic and proactive approach is needed in identifying and managing risks in the supply chain. This study uses the House of Risk (HOR) method to identify, analyze, and design the most appropriate risk mitigation strategy. The HOR approach allows companies to prioritize risk sources based on their severity and probability of occurrence, and determine effective improvement steps. In this study, 20 risks were identified which were grouped into five main categories, namely plan, source, make, deliver, and return, with 14 main risk sources.
