One of its main products, F.JACKET HYC22, has a Part Per Million (PPM) defect rate of 74,000 or 7.4\%, which far exceeds the company?s tolerance limit of 10,000 PPM. This high PPM value can impact production efficiency. This study aims to identify the types of failure that occur, analyze their causes and impacts, determine failure priorities based on risk levels, and provide appropriate improvement recommendations to reduce the failure rate. The methods used in this research are an integration of Fault Tree Analysis (FTA), Failure Mode and Effect Analysis (FMEA), Fuzzy Analytical Hierarchy Process (FAHP), and Grey FMEA (GFMEA). The results show six main types of product failures: hanging thread, improper/double stitch, broken stitch, unbalance, peel of stud, and decal reject. Based on the GFMEA analysis, the highest-priority failure is unbalance, followed by improper/double stitch and hanging thread. The root causes of failure are further analyzed using the Fishbone Diagram, and improvement recommendations are developed using the Five-M Checklist approach. The proposed improvements focus on operator training, equipment maintenance, and better control of machine temperature and pressure. These results are expected to assist the company in reducing the failure rate and improving both production efficiency and product quality.} }
