"PT IGP Internasional Bantul is a manufacturing company engaged in the garment\r\nsector, particularly in the production of doll clothing for the export market. One of\r\nits main products, F.JACKET HYC22, has a Part Per Million (PPM) defect rate of\r\n74,000 or 7.4%, which far exceeds the company’s tolerance limit of 10,000 PPM.\r\nThis high PPM value can impact production efficiency. This study aims to identify\r\nthe types of failure that occur, analyze their causes and impacts, determine failure\r\npriorities based on risk levels, and provide appropriate improvement\r\nrecommendations to reduce the failure rate. The methods used in this research are\r\nan integration of Fault Tree Analysis (FTA), Failure Mode and Effect Analysis\r\n(FMEA), Fuzzy Analytical Hierarchy Process (FAHP), and Grey FMEA (GFMEA).\r\nThe results show six main types of product failures: hanging thread,\r\nimproper/double stitch, broken stitch, unbalance, peel of stud, and decal reject.\r\nBased on the GFMEA analysis, the highest-priority failure is unbalance, followed\r\nby improper/double stitch and hanging thread. The root causes of failure are\r\nfurther analyzed using the Fishbone Diagram, and improvement recommendations\r\nare developed using the Five-M Checklist approach. The proposed improvements\r\nfocus on operator training, equipment maintenance, and better control of machine\r\ntemperature and pressure.
