Taufiq Aji, S.T., M.T., IPM. abstract: Perumda Air Minum Tirta Binangun is a regional company producing Bottled Drinking Water under the AirKu brand. The 240 ml AirKu product has the highest market demand; however, the company failed to meet production targets during several periods in 2024 due to various issues. This study aims to measure the productivity level of the 240 ml production unit, identify the causes of productivity decline, and propose improvement recommendations. The methods used include the Objective Matrix (OMAX) to assess productivity based on eight performance ratios, with weightings determined using the Analytical Hierarchy Process (AHP). Fault Tree Analysis (FTA) is applied to identify the root causes of the lowest performance indicator. Data were collected through observations, interviews, and production documents from January to December 2024. The findings show that the highest performance occurred in August with a score of 664.21, while the lowest was in December at 32.48. The productivity index fluctuated, with the highest increase in August (53.17%) and the steepest decline in December (-82.26%), mainly due to low performance in Ratio 8. FTA analysis indicates the root causes were limited operator knowledge and frequent machine component failures.
