Neglect and mental disorders are two issues that the elderly are vulnerable to. Neglected elderly people with mental disorders require treatment that focuses not only on pathology but also on recovery and strengthening the social functions of the elderly. This study aims to describe the situation of neglected elderly people with mental disorders and analyze the treatment model applied at the Tresna Werdha Social Service Center (BPSTW) Abiyoso Unit. The research used qualitative methods with a case study strategy. Data were collected through semi-structured interviews, observation, and documentation. Informants were selected using purposive sampling techniques from several informants, including the head of the center, social workers, nurses, rehabilitation and social service management, caregivers, and elderly clients. Data analysis was conducted thematically using the Biopsychosocial-Spiritual (BPSS) approach and Strengths-Based Perspective (SBP) as the theoretical framework. The results of the study show that elderly people with mental disorders at BPSTW Unit Abiyoso generally have declining biological conditions, but still show strengths that can be developed as resources. Some elderly people experience cognitive decline, residual symptoms of mental disorders, and limitations in activities and social interactions. Treatment is carried out through collaboration between employees who focus on the biopsychosocial-spiritual conditions of the elderly, which includes meeting basic needs, medication control, spiritual guidance, and strengthening social relationships through group activities. Mental health monitoring of elderly people with mental disorders is routinely carried out in collaboration with the PDHI Hospital. The treatment model is supported by a Strengths-Based Perspective approach, non-discriminatory humanism, positive reinforcement, and corrective measures. Treatment continues despite structural obstacles and limitations in facilities and human resources.
