SP Alumunium Yogyakarta is a company engaged in the manufacturing of aluminum household utensils, one of which is the AL series aluminum frying pan. In its production process, defective products are still encountered, resulting in decreased product quality and financial losses for the company. This study aims to identify the types of defects and the dominant defect in AL series aluminum frying pans, determine the Defects Per Million Opportunities (DPMO) value and Sigma Level, analyze the factors causing defects, and propose improvement recommendations to reduce the number of defective products. The research employed the Six Sigma method with the DMAIC (Define, Measure, Analyze, Improve, and Control) approach, supported by Fault Tree Analysis (FTA), Failure Mode and Effect Analysis (FMEA), and 5 Whys Analysis. The results showed that there were three types of defects: porous defects at 56%, flash defects at 34%, and cracked defects at 11%, with porous defects being the most dominant. The calculated DPMO value was 18,658.55 with a Sigma Level of 3.58. Based on the FTA and FMEA analyses, the main causes of defects were molten metal pouring that did not comply with standard operating procedures (SOPs), damaged or imprecise molds, operator fatigue, inappropriate molten metal temperature, and gas trapped inside the mold. Based on the 5 Whys analysis, the proposed improvement recommendations include the implementation of a frying pan manufacturing SOP that regulates production stages from raw material preparation to product inspection after the casting process.
