description: Productivity is one of the important indicators used to assess the efficiency and effectiveness of a company's performance. UD Surya Tanindo is a rice milling company that has not yet implemented a structured operational performance measurement system, causing productivity evaluations to rely mainly on management experience and observations. In addition, the manual recording system currently used creates delays in data processing, increases the potential for human error, and makes objective productivity monitoring difficult. This study aims to measure the operational performance level of UD Surya Tanindo, identify critical ratios, determine the root causes of problems, and provide improvement recommendations. The methods used in this study include the Analytical Hierarchy Process (AHP) to determine the weight of performance indicators, Objective Matrix (OMAX) to measure productivity levels, Traffic Light System (TLS) to interpret measurement results, and Fault Tree Analysis (FTA) to identify the root causes of operational problems. The results showed that raw material efficiency (R₄) had the highest weight of 0.54949, followed by operational cost efficiency (R₃) with 0.24761, operational productivity (R₂) with 0.12929, and labor productivity (R₁) with 0.07364, with a Consistency Ratio value of 0.0757. The OMAX results indicated an annual average performance indicator value of 444.60. The TLS evaluation showed that 32 assessments were categorized in the red zone, 11 in the yellow zone, and 5 in the green zone. Based on the FTA results, improvement recommendations were proposed through the development of SOPs Preventive Maintenance, Operational Cost Control, and Rice Grain Quality Control.
