This study aims to identify, assess, and design a preventive hazard risk control system to improve safety standards by applying the Hazard Identification and Risk Assessment (HIRA) and Hazard and Operability Study (HAZOP) methods. Validity testing of all risk findings in the field was conducted using the Content Validity Ratio (CVR) method involving 6 field experts (Subject Matter Experts) as expert validators. The results of data processing significantly indicate that there are 23 hazard variables identified within the workshop's operational system. Risks categorized as Extreme were found to be centered in the grinding and engine servicing areas, caused by low worker discipline regarding the OHS culture, particularly in the use of Personal Protective Equipment (PPE), as well as a lack of mechanical assisting tools. As a preventive measure, managerial implications were formulated in the form of comprehensive short-, medium-, and long-term concrete action plans to mitigate these hazards. In conclusion, this study successfully mapped out all critical points in detail and produced practical outputs in the form of a custom OHS logo attribute design and a draft of OHS Standard Operating Procedure (SOP) regulation documents to create a much safer, healthier, and more secure work environment for all mechanics at Bengkel Mobil Tembi.
