Taruna Karya Utama is a manufacturing company engaged in ready-mix concrete production. Activities on the production line at PT. Taruna Karya Utama involve various machinery, materials, and chemicals that potentially pose occupational accident risks, ranging from low to extreme levels. These potential risks are heightened by the lack of a comprehensive risk analysis and low worker compliance with Personal Protective Equipment (PPE) usage. This study utilizes the Hazard Identification and Risk Assessment (HIRA) and Hazard and Operability Study (HAZOP) methods to identify potential hazards, assess risk levels, analyze the causes and consequences of deviations, and provide risk control recommendations. The results indicate that there are 19 validated potential hazards on the production line of PT. Taruna Karya Utama. Based on the risk assessment using the HIRA method, the risks comprise 2 extreme categories with a percentage of 10\%, 6 high risks with a percentage of 32\%, 7 moderate risks with a percentage of 37\%, and 4 low risks with a percentage of 21\%. To minimize potential hazards, risk control actions are implemented using the HAZOP worksheet. The analysis yields 49 proposed control measures referencing the ISO 45001 (2018) hierarchy, consisting of 12 engineering controls, 26 administrative controls, and 11 PPE usage measures.} }
