Taruna Karya Utama is a manufacturing company engaged in ready-mix&#13;
concrete production. Activities on the production line at PT. Taruna Karya Utama&#13;
involve various machinery, materials, and chemicals that potentially pose&#13;
occupational accident risks, ranging from low to extreme levels. These potential&#13;
risks are heightened by the lack of a comprehensive risk analysis and low worker&#13;
compliance with Personal Protective Equipment (PPE) usage. This study utilizes&#13;
the Hazard Identification and Risk Assessment (HIRA) and Hazard and&#13;
Operability Study (HAZOP) methods to identify potential hazards, assess risk&#13;
levels, analyze the causes and consequences of deviations, and provide risk&#13;
control recommendations. The results indicate that there are 19 validated&#13;
potential hazards on the production line of PT. Taruna Karya Utama. Based on&#13;
the risk assessment using the HIRA method, the risks comprise 2 extreme&#13;
categories with a percentage of 10%, 6 high risks with a percentage of 32%, 7&#13;
moderate risks with a percentage of 37%, and 4 low risks with a percentage of&#13;
21%. To minimize potential hazards, risk control actions are implemented using&#13;
the HAZOP worksheet. The analysis yields 49 proposed control measures&#13;
referencing the ISO 45001 (2018) hierarchy, consisting of 12 engineering&#13;
