The Family Hope Program (Program Keluarga Harapan/PKH) not only distributes financial assistance but also requires facilitators who assist Beneficiary Families (Keluarga Penerima Manfaat/KPM) in accessing services and building self-reliance. This study aims to analyze the roles and outcomes of PKH facilitators in advancing SDG achievement in Kemantren Kotagede, Yogyakarta. This research employed a descriptive qualitative method conducted in Kemantren Kotagede, covering the sub-districts (kelurahan) of Purbayan, Rejowinangun, and Prenggan. Informants were selected through purposive sampling, consisting of three PKH facilitators and nine KPM with varying participation conditions. Data were collected through in-depth interviews, observation, and documentation, then validated using source and method triangulation, and analyzed using the interactive model of Miles, Huberman, and Saldaņa through the stages of data reduction, data display, and conclusion drawing. The findings indicate that PKH facilitators carry out facilitative, educative, representative, and technical roles. These roles are reflected in facilitating access to education, health services, and social security; resolving administrative issues; conducting P2K2 (Family Development Sessions); strengthening family financial management; and connecting KPM with empowerment programs and cross-agency services. The outcomes of this facilitation are evident in improved access to education and health services, increased family attention to nutrition and childrearing, enhanced ability to manage household needs, more focused utilization of assistance, and encouragement toward productive enterprises and independent graduation (graduasi mandiri) from the program. These contributions support poverty alleviation, healthy living, quality education, and reduced inequality; however, behavioral change and self-reliance among KPM remain uneven, indicating that continued facilitation is still necessary.
