As a small and medium-sized enterprise (SME), Ngizan Apparel faces various risks throughout its supply chain activities that may disrupt production processes and order fulfillment, including delays in raw material supply, production delays, and defective products. This study aims to identify the risks that hinder the sports jersey supply chain at Ngizan Apparel and to propose appropriate mitigation strategies based on the identified priority risks. The study employed the Supply Chain Operations Reference (SCOR) model to map supply chain activities and the House of Risk (HOR) method to identify and evaluate supply chain risks. In HOR Phase 1, a total of 14 risk events and 24 risk agents were identified, with 14 risk agents selected as priorities based on the 80/20 Pareto analysis. Dependence on a single supplier was identified as the risk agent with the highest Aggregate Risk Potential (ARP), followed by delays in raw material delivery. In HOR Phase 2, five preventive actions (PAs) were formulated to address the prioritized risk agents, namely supplier diversification and supplier performance evaluation, preventive maintenance, structured training for production workers, the implementation of a Google Form-based ordering system, and the addition of production personnel. The results of the Effectiveness to Difficulty Ratio (ETD) analysis indicated that structured training for production workers is the highest-priority mitigation strategy. Therefore, it is recommended as the first strategy to be implemented by Ngizan Apparel to sustainably reduce potential risks within its supply chain. %Z Dr. Ir. Yandra Rahardian Perdana, S.T., M.Eng.
