      "abstract": "Ngizan Apparel is a manufacturing company engaged in the production of sports\r\njerseys in Yogyakarta. As a small and medium-sized enterprise (SME), Ngizan\r\nApparel faces various risks throughout its supply chain activities that may disrupt\r\nproduction processes and order fulfillment, including delays in raw material\r\nsupply, production delays, and defective products. This study aims to identify the\r\nrisks that hinder the sports jersey supply chain at Ngizan Apparel and to propose\r\nappropriate mitigation strategies based on the identified priority risks. The study\r\nemployed the Supply Chain Operations Reference (SCOR) model to map supply\r\nchain activities and the House of Risk (HOR) method to identify and evaluate supply\r\nchain risks. In HOR Phase 1, a total of 14 risk events and 24 risk agents were\r\nidentified, with 14 risk agents selected as priorities based on the 80\/20 Pareto\r\nanalysis. Dependence on a single supplier was identified as the risk agent with the\r\nhighest Aggregate Risk Potential (ARP), followed by delays in raw material\r\ndelivery. In HOR Phase 2, five preventive actions (PAs) were formulated to address\r\nthe prioritized risk agents, namely supplier diversification and supplier\r\nperformance evaluation, preventive maintenance, structured training for\r\nproduction workers, the implementation of a Google Form-based ordering system,\r\nand the addition of production personnel. The results of the Effectiveness to\r\nDifficulty Ratio (ETD) analysis indicated that structured training for production\r\nworkers is the highest-priority mitigation strategy. Therefore, it is recommended as\r\nthe first strategy to be implemented by Ngizan Apparel to sustainably reduce\r\npotential risks within its supply chain.",
