Abstract

Factors that cause product defects are problems that often arise in the production process so they can affect product quality. Consumer expectations and quality standards in the company are a reference for a company to improve quality standards, therefore a good quality control process is needed to reduce defective products that occur at PT. Indorama Teknologies Complex. Faulty products cause a comparison of the target with the realization of a decrease in revenue and profit and loss. The process of identifying the cause of the defect uses the Failure Mode Analysis (FMEA) and Fault Tree Analysis (FTA) methods. This study aims to analyze and identify the causes of defects in NE 30s polyester yarn products. The calculation of the production amount data and the defect number data are used to determine the order of the highest defect type to the lowest defect type using a Pareto chart. Then to analyze and identify the factors that cause the disability is carried out using a fishbone diagram. The FMEA assessment was used by 3 FMEA criteria which included severity, occurrence, and detection based on a questionnaire from 5 sources, then calculations were carried out by exploring these three criteria and producing a Risk Priority Number value (RPN). The root cause of the defect will be analyzed using FTA by making a tree diagram. Repairs were carried out to overcome the large number of defective products that occurred using the 5W+1H repair proposal given to improve the reputation of PT. Indorama Teknologies Complex. Based on the results of the study, the highest RPN value from the cause of the defect that occurred was 448 at the defect break and it is necessary to make improvements by providing an understanding to the operator regarding more careful thread connection. By using FMEA and FTA, the level of product defects is proven to be minimized to meet consumer satisfaction.
