Abstract
CV. 1Souvenir is a manufacturing company engaged in the production of plaques using various types of raw materials such as wood, acrylic, metal, and others. The production activities at CV. 1Souvenir carry potential risks of occupational accidents, ranging from low to extreme levels. Therefore, the implementation of the Occupational Health and Safety Management System (OHSMS) is crucial to minimize these risks. This study employs the Hazard Identification and Risk Assessment (HIRA) and Hazard and Operability Study (HAZOP) methods to identify risks, assess their levels, and provide recommendations for risk control based on the analysis of causes and impacts of each identified risk. The results show that there are 31 potential occupational hazards identified in the machinery area of the production division at CV. 1Souvenir Indonesia. Based on the HIRA analysis, the distribution of risk levels is as follows: extreme risk at 13%, high risk at 39%, medium risk at 35%, and low risk at 13%. Risk control efforts were carried out to reduce potential hazards using the HAZOP worksheet. Risk control measures were determined by following the OHSAS 18001 risk control hierarchy, which includes elimination, engineering controls, administrative controls, and the use of Personal Protective Equipment (PPE). The analysis results identified 65 control actions, consisting of 3 substitution actions, 21 engineering controls, 22 administrative controls, and 19 PPE-related actions.
