Abstract

This study examines the case management practices carried out by medical social workers at Prof. Dr. R. Soeharso Orthopedic Hospital Surakarta in the rehabilitation of paraplegia patients who experience biopsychosocial disruption due to sudden trauma. The research applies a case management approach grounded in the theoretical framework of Roberts and Greene, George L. Engel’s biopsychosocial model, and the professional functions of medical social workers as proposed by Marianne R. Woodside and Tricia McClam. These theories serve to strengthen previous studies on the topic. A descriptive qualitative method was employed using a case study approach, with data collected through interviews and documentation. The research subjects included all medical social workers at the hospital and three selected paraplegia patients with varying backgrounds of injury. The findings reveal that case management is implemented in a structured and holistic manner through seven strategies representing the stages of case management: engagement, assessment, planning, intervention, evaluation, and termination. Patients received tailored biopsychosocial interventions, including access to social security, psychosocial support, family empowerment, and discharge preparation. Medical social workers performed eight core professional functions as case managers, namely: recordkeeper, counselor, planner, consultant, advocate, broker, service coordinator, and problem solver. However, the implementation of case management faces key challenges, particularly the limited number of professional staff and the lack of institutional policy support for follow-up monitoring, which hinders service continuity after discharge. Therefore, this study recommends strengthening institutional policies in optimizing resource allocation, developing post-service monitoring systems, and enhancing the professional capacity of medical social workers to ensure more comprehensive, adaptive, and sustainable case management practices.
