Abstract

Colorectal cancer was the third most frequently diagnosed cancer in Indonesia in 2024. Conventional cancer treatment modalities have limitations in protecting organs at risk (OARs) from radiation exposure, thereby encouraging the development of more effective therapeutic approaches such as Boron Neutron Capture Therapy (BNCT). This study aimed to develop the geometric models of colorectal cancer, OARs, and the Beam Shaping Assembly (BSA), as well as to evaluate the effectiveness of two boron delivery agents, boronophenylalanine (BPA) and sodium borocaptate (BSH), in BNCT for colorectal cancer. A Monte Carlo simulation approach was employed using PHITS version 3.35. The organ geometry was based on the ORNL mathematical phantom, while the organ material compositions followed ICRP Publication 145. A 30 MeV neutron source from the Kyoto University Research Reactor (KUR) was used, and boron concentrations ranging from 50 to 150 μg/g of tumor tissue were investigated. The simulation results showed that the total tumor dose rate increased linearly with increasing boron concentration. BPA produced total dose rates ranging from 2.22 × 10⁻² to 6.21 × 10⁻² Gy/s, whereas BSH produced total dose rates ranging from 8.04 × 10⁻³ to 2.06 × 10⁻² Gy/s. The irradiation time for BPA ranged from 13 to 38 minutes, while that for BSH ranged from 40 to 104 minutes. The equivalent doses delivered to the OARs remained below the safety limits for all organs in the BPA simulations. In contrast, the equivalent dose to the urinary bladder exceeded the safety limit in the BSH simulations. These findings demonstrate that BPA is more effective and safer than BSH as a boron delivery agent for BNCT in the treatment of colorectal cancer.
