Abstract

The company always tries to improve the quality of its products in order to gain the trust of customers. Quality control that is carried out properly and effectively can maintain and improve the quality of the company's products. Drive sprocket AM is one of the products manufactured by PT. Aiko Tekindo Sakti. The Failure Mode and Effect Analysis (FMEA) method is used to reduce the failure of drive sprocket AM products. FMEA aims to determine the priority of potential failures by calculating the RPN value based on the multiplication of the level of severity, occurrence and detection. However, the RPN value in the FMEA method has the potential to produce the same value, but does not consider different levels of failure factors. Therefore, the GFMEA method is used to obtain a more accurate RPN value because different failure factor values can be considered. GFMEA is an integration between traditional FMEA methods, FAHP and gray theory. In this study, 17 types of failures were found in the production process of the drive sprocket AM product based on the results of observations and interviews with the company. Then, determine the highest GRPN value over the critical value limit of 0.5 so that 3 types of priority failures are obtained which are given suggestions for corrective actions. This failure type includes round out +/- of 0.358; TBD +/- from 0.368; gear defects of 0.462. Based on the journal and the results of discussions with the company, recommendations for corrective action were obtained, including re-emphasizing and giving a firm stance to carry out the production process according to SOP, checking and maintaining the machines, and conduct training related to chain inspection, use measuring tools properly and machine settings periodically.
