Abstract

PT XYZ is a company that has considerable production process activities every year with potentially high hazard risks. The implementation of SMK3 is needed to reduce and prevent work accidents. This research is used to analyze the potential hazards that may arise in the production department of PT XYZ as an initial form of SMK3 implementation. This research uses the Failure Mode and Effect Analysis (FMEA) and Hazard and Operability Study (HAZOP) method approaches to conduct risk identification, risk assessment and analysis, then given an analysis of the causes, impacts, and risk control. The results showed 34 potential hazards in the production department of PT XYZ. Where the FMEA method risk assessment obtained 6% of them at the extreme level, 15% at the high level, 41% at the moderate level, and 38% at the low hazard level. While the HAZOP method obtained 21% of them at the extreme level, 24% at the high level, 26% at the moderate level, and 29% at the low danger level. The risk assessment is then carried out weighting the assessment criteria using the Fuzzy Analytical Hierarchy Process (FAHP). Where the risk assessment obtained 7 critical risks with the largest RPN value and Risk Level of Fuzzy AHP results from the priority ranking of fuzzy AHP weighting. Risk control measures using the help of FMEA and HAZOP worksheets based on risk priorities obtained amounted to 64 actions. Risk control is provided by following the OHSAS 18001 risk control hierarchy and AS/NZS 4360 strategies to reduce risk severity, namely elimination, engineering, administration, and use of PPE (Personal Protective Equipment). This action has an impact on saving company costs to anticipate work accidents in the production department of PT XYZ.
